If you have been approved for a home care package, then there are many additional options now available to you which can assist you to live at home as comfortably as possible. Now that you know the allocated funds you have available as a subsidy from the Australian Government, the next step is to organise the help you need as quick as possible.

At Country Home Services, we can help you through this next stage through taking you through the following steps.

  1. Fees

Ensuring that you understand the overall fees including the contribution required by you towards your home care package. The fees consist of three equal parts:
– Basic Daily Fee

  1. Selecting the right service provider

When you have just received the Home Care Package subsidy, it is important that you have access to reliable, trustworthy providers who are arranged in a timely manner so that you can receive the help you need immediately.

At Country Home Services, we will ensure that you are matched with providers who are local to where you live and are proven, trustworthy and insured providers who share our core philosophy of providing exceptional customer service to ensure that you are taken care of in the best possible way.

Services which we can assist with providers for you, and are typically covered under the Home Care Package, include:
– household tasks and cleaning
– respite
– personal assistance
– social support
– garden and home maintenance
– meal preparation
– in home nursing
– health care & therapeutic services
– transport
– social outings & community connection

Our trained professionals can also advise on any additional services and/or funding supplements which could be beneficial to you. This includes applications for further funding supplements such as the dementia and cognition supplement, oxygen supplement, enteral feeding supplement, veterans’ supplement and viability supplement.

Once you have chosen the approved providers that you wish to work with, we can assist you to complete a Home Care Agreement. This agreement will outline the services that will be delivered and your rights during the time that the services are provided. Our Country Home Services Team will then guide you through the process of submitting your providers to the government. Once this has been completed your services will begin.

  1. Ongoing support

At Country Home Services, we are committed to ensuring that you are provided with ongoing support as you need it. We will provide ongoing support including checking in to see how you are and whether the Home Care Package you have is meeting all of your individual needs.
