Home Care Packages provide assistance within the home where it is needed for many everyday tasks, as well as situations in which the needs of the person needing the help become intense or complex. Support is available with services including bathing, hygiene and grooming, nursing, personal care services which relate to health and mobility, meals and food preparation, cleaning and laundry, transport and more!

Services available through Home Care Packages
Home Maintenance
Home maintenance and repairs, and garden maintenance which is required to keep the house safe.
Meals
Shopping and assistance with delivery of meals to ensure that you can continue to eat healthy.
Personal Care
Personal care services including bathing, grooming and personal hygiene.
Nursing
Nursing services can be provided to assist with treating and monitoring medical conditions at home.
Cleaning, laundry and other chores
Cleaning duties, assistance with washing and drying clothes and other chores as are required.
Podiatry, physiotherapy and other therapies.
Podiatry, physiotherapy and any other therapies which are required to assist with mobility and movement.
Transport
Transport is available to ensure you can still travel to any appointments and activities attended within the community.
Social Support
Social support including assistance with attending external social events.

A Home Care Package offers more choice than ever before. However, how much choice depends on your particular service provider. At Country Home Services we utilise independent contractors to deliver services rather than employed staff. By using independent contractors, we have access to a large variety of qualified service providers while still providing quality peace of mind. Our independent contractors undergo ongoing quality auditing, training and all have a criminal history check. More importantly, they are matched to meet the individual’s needs and shared interests.
Home Care Packages
Most importantly, our Home Care Packages are individual suites of services to support your lifestyle. Our purpose is to support your social, physical, emotional, cultural and spiritual needs which means the scope of a Country Home Services Home Care Package is quite exceptional.
From there, the My Aged Care contact centre may refer you for a home support assessment, where a Regional Assessment Service (RAS) assessor will contact you to arrange an in-home assessment.
How can you see if you are eligible?
A Home Care Package is a package of services funded by the Australian Government. An assessment is required to check if you are eligible for a Home Care Package. This is conducted by the Aged Care Assessment Team.

How do you apply?
There are a couple of ways you can apply for the Home Care Packages.
Call My Aged Care
You can call My Aged Care on 1800 200 422. From there, the My Aged Care contact centre will ask you some questions and if eligible, you will be referred for a home support assessment, where someone from the Aged Care Assessment Team (ACAT) assessor will contact you to arrange an in-home assessment.
Online
You can use the My Aged Care online form to apply which will only take approximately 15-20 minutes. If you get stuck completing this application then you are welcome to contact us and we can assist you. We are here to ensure you are taken care of and understand what is available for you.
What do you do once approved?
Once you have been assessed for a Home Care Package (HCP), you will receive a letter from the person who assessed you outlining your eligibility and HCP Level (1 through to 4.) If you are eligible for a HCP, you will automatically get placed on a national waiting list, and once a HCP becomes available, you will receive a letter that assigns you a Home Care Package.
This is the time to then contact us. You should then act on this as soon as possible as you only have a certain time to choose a provider and enter into a Home Care Package Agreement.
So, while you are waiting to be assigned a HCP, you should start thinking about what a HCP could look like for you. Think about things such as:
- The types of services that you need.
- The type of services that will suit your lifestyle.
- The fees and charges for services and other products.
If your circumstances improve while you are on the waiting list, you can opt-out of the waiting process at any time. Your referral remains valid until your needs change.
